As the Aged Care Coordinator, you will play a pivotal role in overseeing and optimizing the care services provided to our elderly residents. Your responsibilities will include:

- Conducting comprehensive assessments of residents' needs and preferences to create individualized care plans.
- Coordinating a multidisciplinary team of healthcare professionals to ensure seamless care delivery.
- Monitoring the well-being and health status of residents, proactively addressing any concerns.
- Collaborating with families and residents to ensure their preferences and choices are respected and integrated into their care plans.
- Implementing quality assurance measures to maintain high standards of care and adherence to regulatory guidelines.
- Organizing and facilitating care conferences and meetings to review resident progress and care plan effectiveness.
- Providing support and guidance to care staff to ensure residents receive compassionate and person-centered care.
- Managing resources effectively to meet the needs of residents and maintain operational efficiency.

**Qualifications**:

- Bachelor's degree in Nursing, Aged Care Management, Health Services, or a related field. A Master's degree is a plus.
- Proven experience in aged care coordination or a similar healthcare management role.
- Excellent communication and interpersonal skills.
- Strong leadership and decision-making abilities.
- Knowledge of aged care regulations and quality standards.
