Just Better Care Central Coast is looking for experienced Community Support Professionals to provide person centered support to customers in their homes and the community across Central Coast area.

Our team of Community Support Professionals provide our customers with a high quality of care that is respectful, dignified, safe and encourages independence whilst adhering to the individual’s support plan. Our Community Support Professionals provide crucial care and support to customers in all aspects of their daily life.

The Role
- Developing strong trust based relationships with customers and delivering person centered care and support
- Supporting customers to meet their daily living and lifestyle requirements as per their care plan, including personal care and working with customers with complex and/or challenging needs
- Providing support at home with health conditions and daily tasks of living such as washing, dressing, grooming, eating, shopping and transport
- Supporting customers in the use of aids and personal equipment including: standing and walking frames, wheelchairs, manual & electric hoists, slide sheets and moving boards, hearing aids & other physical aids
- Maintaining a clean, comfortable, safe and secure environment for customers which may include undertaking light domestic duties or meal preparation
- To help in the promotion of a customers mental and physical activities such as through communication and engagement, going out into the community, reading and writing, hobbies and recreation
- Support customers to voice their opinions, needs and choices and ensuring customer rights are upheld
- Ensuring all documentation is completed in a timely & accurate manner
- Monitoring the well-being of customers and reporting on the progress of customers to the Service Coordinators or Package Manager.

To be considered for this role, you will need the following:
- Current NDIS Worker Screening Check
- Current Local Driver’s License to travel between customer’s homes
- Current First Aid certificate
- Desirable - Certificate III in Aged Care/Disability Care/Individual Support.
- Reliable motor vehicle (including valid registration and comprehensive insurance)
- Mobile phone with internet connection compatible with Just Better Care requirements (Schedule and Intranet requirements)
- Previous employment experience in community care, disability care or health/aged care.
