**What you'll be doing**
- Providing personalised support and assistance to families, individuals and children with disabilities or complex needs
- Helping clients access community services and resources to promote independence and improve quality of life
- Coordinating and facilitating group activities and programs to encourage social inclusion
- Maintaining detailed client records and reporting on progress to ensure high-quality care
- Collaborating with a multi-disciplinary team of professionals to develop and implement tailored support plans
- Advocating for clients' rights and addressing any concerns or issues that arise
- Assist the work of Social Workers and welfare workers by providing services and support to families

**What we're looking for**
- Relevant qualifications in community services, social work, or a related field
- Minimum 2 years' experience in a similar family support or disability support role
- Excellent communication and interpersonal skills, with the ability to build trust and rapport with clients
- Strong problem-solving and decision-making skills, with a solution-focused approach
- Passion for making a positive impact in the lives of vulnerable individuals and families
- Ability to work collaboratively within a team and across multiple stakeholders
- Willingness to undergo a National Police Check and Working with Children Check

Pay: $36.00 - $39.00 per hour

Expected hours: 38 per week

Ability to commute/relocate:

- Perth WA 6030: Reliably commute or planning to relocate before starting work (required)

**Education**:

- Certificate I - IV (required)

**Experience**:

- Family Support : 2 years (required)

Licence/Certification:

- Australian Federal Police Clearance (required)
- Working with Children Check (required)

Work Authorisation:

- Australia (required)
