**About the role**

This role is responsible for leading the local network of Neighbourhood teams, to support clients to live at home for longer and stay connected to their community, in this role, you will ensure that the local Neighbourhood teams foster strong relationships and connections with each client and their circle of support to make a real difference in the delivery of person-centred Home and Community Care services.

**About you**
- Tertiary or Diploma qualification in a relevant field, such as Aged Care, Health, Business and/or working towards or equivalent years of experience
- Five or more years' experience in supervising a team, and nurturing them through ambiguity and change
- Significant understanding for client needs, to convert these into positive service delivery outcomes
- Passion for social change and contributing to an organisation of influence for vulnerable, at risk and disadvantaged members of the community
- Current driver's licence and access to a comprehensively insured vehicle
