Exciting opportunity to provide Psyhco-social support under the HOPE team. Position entails training under the Beyond Blue model to provide Psycho-social support for people who are identified as having presented with a suicidal or self-harm attempt, or suicidal or intentional self-harm ideation under the Monash Adult Mental Health Program Hospital Outreach Post-suicidal Engagement (HOPE).
The Psychosocial support role provides non-clinical care and practical support to individuals for up to 12 weeks following a suicide attempt.

Monash Adult Mental Health Program Hospital Outreach Post-suicidal Engagement (HOPE) initiative aims to provide follow-up care and support for people who traditionally would not have received such intervention immediately after hospital discharge, with broader referral pathways and tailored psychosocial support. The aim of the initiative is to prevent avoidable suicide attempts and reduce the number of completed suicides by providing targeted psychosocial support services to a cohort of people in an assertive manner in the community for up to 3 months from the time of discharge Monash Health is excited to announce new Deputy Nurse Manager positions within its Emergency Departments

**About The Role**
In this role you will provide community based support that enables consumers to build self-resilience and capacity to self-manage social, economic and environmental factors that contributes to suicidality.
Provide support to the consumer through a meaningful approach to care that assists consumers to stay safe and connect with essential services in the first 12 weeks following discharge from hospital after a suicide attempt or a suicidal crisis.
The psychosocial support worker provides tailored assertive outreach to support the individual to recover from a crisis In collaboration with the consumer, identify need and provide information, as well as link the consumer with essential services in the first 12 weeks that is meaningful to the consumer
Work collaboratively with the multidisciplinary team to ensure the delivery of a positive and safe experience to at-risk consumers.
Contribute to identifying safety plan with consumer while on HOPE
Develop a sound understanding of crisis intervention and management skills, and the ability to escalate concerns if deterioration occurs.
Respond to and support the psychosocial needs of consumers of the HOPE program as identified in collaboration with the consumer.
**About You**
You have an ability to build supportive and respectful relationships with individuals experiencing distress, including family members and carers.
You have demonstrated experience in, and sound understanding of the personal, family and social issues associated with suicidal ideation and attempts, including groups identified to be at increased risk of suicide or experiencing a suicidal crisis.
You have the ability to establish strong, effective working relationships. I.e. Ability to liaise, consult and
work with a broad range of consumers, carers/families, health professionals, community
agencies and organizations
