**The role**

This role is offered as a Fixed Term Contract until July 2028. This role is based in Bendigo.

**Responsibilities**

The Dementia Support Coach will facilitate on the ground support, implementation of relationship-based support strategies, modelling and coaching with clients, carers and care staff.
- Work collaboratively with peers to develop the skills, expertise and/or applied knowledge of aged care staff and carers to understand the person living with dementia and develop best practice support and care.
- Support the implementation of the recommendation plans on the ground, drawing on modelling and coaching techniques, and seek guidance from Team Leader, Dementia Consultant and Senior Consultant as required.
- Facilitate and support the trial and implementation of brokerage items with clients and their carers, and seek guidance from Team Leader, Dementia Consultant and Senior Consultant as required.

**What do we need from you?**
- Strong organisational abilities, including effective time management and ability to prioritise in a busy work environment and competing demands.
- The ability to work both independently and as part of a team.
- Demonstrated commitment to excellence in customer service principles including conflict resolution to ensure the consistent provision of exceptional service to internal and external customers
- A current driver’s licence and safe driving record
- Willingness to travel and work intrastate and interstate to support the delivery of DC servicesQualification in relevant field; Allied Health assistant, Diversional Therapy, Enrolled Nursing (EN), Aged Care, Cert III or IV in Individual Support / Leisure and Health or similar highly desirable
