Colbrow Care provides premium support services for older Australians receiving Aged Care at home, children and adults who have physical, sensory and/or intellectual disabilities, as well as children and adults injured in road traffic accidents and adults injured in the workplace.
For many Australians living with a disability, reduced mobility and/or cognition, in-home care and support allows them to live comfortably in their own home and actively participate within their community.
Colbrow Care places a strong emphasis on maintaining our client’s privacy, earning their respect, and preserving their dignity.
**ABOUT THE ROLE**

We are looking for a new addition to the program of an NDIS participant who lives in **WHITTLESEA**. She is a kind and very independent women who needs additional support with her daily lifestyle. There are multiple shifts available, 7 days a week. These services are for two hours, three times a day.
Service times are as follows 7 days a week.
0830 - 1030
1330 - 1530
1900 - 2100
You will be responsible for the delivery of personalised support services that enhance our clients' independence, social and community participation and helps them achieve their goals in accordance with their care plans.
You will support each client to maintain their independence and dignity by providing assistance with activities of daily living such as:
- _ Personal care_
- _ Social Support_
- _ Meal Preparation_
- _ Support with exercise_
- _ Community Access_
- _ Transport_
- _ Medication Monitoring_
- _ Domestic Assistance_

To be successful in this role, you will have **AND/OR** be willing to obtain the following:
- _ Certificate III/IV in Community Care, Individual Support or Disability_
- _ Proven experience in this or a similar role/field_
- _ Current Working with Children's Check_
- _ Current NDIS Worker Screening Check (includes Police Check)_
- _ First Aid Certificate (HLTAID011) and CPR (HLTAID009)_
- _ Mandatory competencies (as required by Colbrow Care)_
- _ Valid working rights in Australia_
