THE ROLE
The Targeted Care Packages (TCP) Advisor plays a key role within the West Division Flexible packages team by supporting the implementation, coordination and monitoring of activities associated with individualised and targeted packages to improve outcomes for children in care. Working alongside Placement and Support Planning Teams and Child Protection, the TCP Advisor will help identify children where a TCP may prevent their entry to residential care or support their transition to a more suitable living arrangement and take responsibility to lead the development of a flexible funding package to achieve this. You will use your strong planning, coordination and stakeholder relationship skills to design and implement tailored supports to meet the needs of individual children, their families and carers, providing flexible and innovative placement prevention and out of home care solutions for children. In addition, you'll play a key role in overseeing and monitoring the implementation of the package, working closely with agencies that deliver the TCP's to ensure that goals are achieved and strong financial oversight occurs.
ACCOUNTABILITIES INCLUDE

- Undertake and manage active TCP processes for children exiting residential care or at risk of entering residential care.
- Liaise and work in partnership with internal and external stakeholders to facilitate the resolution of client and placement issues.
- Undertake or support the necessary work involved in preparing a panel presentation for a child and the related follow up.
