As an approved service provider under the NDIS, we strive to offer personalized support tailored to the unique needs of each individual.
We are seeking a compassionate and dedicated Dementia Support Worker to join our team. The primary role of the Dementia Support Worker is to provide high-quality care and support to individuals living with dementia, facilitating their engagement in activities and promoting their overall well-being. The position operates within the framework of the NDIS guidelines to ensure the provision of person-centered, goal-oriented support.
Key Responsibilities:
- Provide one-on-one support to individuals living with dementia, both in their homes and in community settings.
- Assist clients with personal care activities, including bathing, dressing, and grooming.
- Foster a safe and supportive environment for clients, promoting their independence and dignity.
- Develop and implement individualised care plans based on clients specific needs and goals.
- Engage clients in meaningful activities, such as social outings, cognitive exercises, and recreational pursuits.
- Monitor and document clients' progress, noting any changes or concerns in their condition.
- Collaborate with clients' families, healthcare professionals, and other support workers to ensure a coordinated approach to care.
- Provide emotional support and companionship to clients, promoting social interaction and mental well-being.
- Maintain accurate records and documentation in accordance with NDIS requirements.
Qualifications and Experience:
- Certificate III or IV in Individual Support/Aged Care/Disability or a related field.
- Previous experience in providing care and support to individuals with dementia is highly desirable.
- Familiarity with the NDIS framework and guidelines is an advantage.
- Strong interpersonal and communication skills, with the ability to build rapport with clients and their families.
- Compassionate and patient approach, with the ability to empathise with individuals living with dementia.
- Ability to work independently, prioritise tasks, and adapt to changing circumstances.
- Understanding of dementia-related behaviors and the ability to effectively manage challenging situations.
- Valid First Aid and CPR certifications
- Current driver's license and access to a reliable vehicle
- Valid Police Check
