**About the Opportunity**

As Real Community Services continues to grow, so do the opportunities to work with us. We currently have multiple positions available in multiple regions, to provide support to our participants to meet their individual goals.

We are currently seeking experienced and enthusiastic support workers to join our dedicated team to support a diverse range of participants in Palmview, Caloundra, Moffat Beach and Sippy Downs areas. Shifts in the Nambour area may also be required.

In this role you will be assisting individuals with various disabilities, helping them prioritise tasks as well as foster a supportive environment, this will help promote their independence as well as their personal growth.

**Other responsibilities may include**
- Provide individual support to achieve individual goals
- Support the participant in the community
- Assistance with domestic duties
- Assistance with the preparation of nutritious meals and supporting a healthy lifestyle
- Providing accurate and detailed incident reports, shift notes and shift handovers
- Maintaining participant records
- Medication Management (Diabetes medication experience highly desirable)
- Manual Handling

**Qualifications and Skills**
- Cert 3 Individual Support or equivalent
- And/or minimum 1 year experience supporting clients with High Physical Support Needs, Psychosocial disabilities and/or complex behaviours is highly desirable
- Valid Drivers licence and own car is essential
- Current Paid employee Blue Card
- Current Paid employee Yellow Card/NDIS Worker Clearance
- Current First Aid & CPR
- NDIS ‘Worker Orientation’ module (or willing to obtain before employment)
- Full Australian working rights
- Smart phone with internet access
- Two current (within the past 2 years) professional references from a supervisor will be required
