**Our Purpose**
United for Care offers independent living for members of the community to live in individual apartment accommodation with 24 hour support in our Vertical Villages. The team at United for Care are passionate and dedicated to innovation in the disability sector with the purpose of improving the quality of life for the vulnerable and people living with disability.

**About the Position**

The Mental Health Support Worker will provide support with people living with complex trauma, and psycho-social challenges and needs aligned with their individual program plan.

Working with external care teams and families, the role is responsible for providing high-quality, responsive, and person-centred support to assist participants enhance their everyday living skills, gain access and participation in community to achieve their goals, enhance their quality of life and physical and mental health.

**The key responsibilities are to**:
1. Provide support with daily living skills and community activities to meet individual needs of our participants which may include but not limited to housekeeping, personal hygiene, administering medication and transportation to appointments and community activities.

2.Work with the participant’s clinical team, attend training and regular updates to ensure all treatment plans are met and implemented to a high standard of care.

3.Provide short-term focused interventions, psychoeducation and mentoring to participants.

4.Communicate effectively and appropriately with external stakeholders and family members.

5.Complete mandatory online training in a timely manner and undertake specific skills and client training as required.

6. Facilitate community inclusion and participation, dependent on individual needs, abilities and preferences, and support participants to establish and maintain relationships and interests through access to community activities and services.

**About You**

You will thrive and succeed in our team by bringing the following experience to this position:

- Qualification in mental health, psychology, social work or related field (desirable)
- Preferable 2 years' experience in mental health and complex/challenging behaviours
- including with a disability, mental illness, community services, aged care, residential or rehabilitation environment.
- A clear understanding of professional boundaries and restrictive practices.
- Values consistent with person-centred ways of working that supports the person’s choice and control.
- Demonstrated ability to make sound decisions under pressure and de-escalate crises.
- Effective written and verbal communication skills and a demonstrated ability to write clear case notes.
- Have sound emotional intelligence and communication skills including patience, empathy, tact and reflective listening.
- NDIS Worker Screening Clearance (obtained from Service NSW)
- Current First Aid Certificate
- Unrestricted Australian Driver’s License
- Covid Vaccination Certificate(strongly encouraged)
