We have a 0.53EFT permanent part time role available within the Hospital Admission Risk Program (HARP). HARP provides specialist treatment, care planning, education, and support to help people with chronic and complex health issues to manage independently and reduce their risk of a hospital admission. Services are delivered to the clients/carers in their homes, community health services or outpatient settings; through face-to-face, telephone or telehealth modalities. HARP LInCS (**L**iaison, **In**take and Virtual **C**are Coordination **S**ervice) liaises closely with referring services to support the referral and intake of people who fit the HARP service criteria and provides a virtual care coordination service for eligible HARP clients. This role aims to contribute best practice patient care within the delivery of HARP LInCS including the triage of targeted patient cohorts, appropriate referral workflows, and virtual care post discharge follow up. HARP LInCS works closely with General Medicine and operates 7 days per week. This position is based at City Campus, and participation in a weekend roster is required as part of this role.

**Your new role**

We are looking for passionate and dedicated individuals to join our HARP LInCS (Virtual Care) service with the HARP Complex Care team. As part of this fabulous and highly experienced team, you will:

- Conduct comprehensive client assessments to identify health and psycho-social issues and risks using a high level of clinical expertise
- Provide best practice patient care through developing and implementing care plans to address health and psycho-social issues, through coordination of care with clients/carers, HARP clinicians, GPs and other services
- Actively support clients to make informed decisions about their treatment options and ongoing care needs, including appropriate medical follow up as required.
- Ensure clients receive adequate health education / information in an appropriate and accessible format to support their post discharge care needs.
- Providing a responsive and informed first point of contact for referrers to facilitate referrals to the HARP service
- Triaging recently discharged clients who are at high risk of re-presenting to hospital and facilitating appropriate referral workflows

**About you**:

- Experience in complex needs assessment, care planning and coordination for clients with chronic disease and complex psychosocial needs
- Understand factors contributing to hospital admission in clients with complex needs
- High level knowledge of Community Services
- Demonstrated ability to be flexible, energetic, innovative, and work cooperatively in a multidisciplinary team as well as autonomously
- Competent IT user, with well-developed written and verbal communication skills
- Qualifications in either Registered Nurse or Allied Health equivalent
