At United for Care our focus is on helping people with disabilities to achieve fuller, better lives. United for Care offers independent living opportunities for members of the community living with complex disabilities to live in individual apartment accommodation with 24 hour support._

**Position Overview**

Reporting to the Integrated Services Lead, and working collaboratively within the Coordination Support team, you will be responsible for coordinating the provision of services for UFC clients, resolving situations as they arise, and ensuring ongoing communication with staff, clients, families and other stakeholders. Equipped with your experience in the Disability Services industry, you will be motivated and proactive, with the ability to rapidly learn the ropes and ‘hit the ground running'. Your skills extend to identifying and building relationships with referral partners.

**Key **Responsibilities**
- Deliver coordination of support services to participants in accordance with NDIS practice guidelines.
- Work with participants and their support networks to assist them with all aspects of their NDIS plan and navigate the NDIS to determine how their funds will be most appropriately spent and what services and supports need to be engaged.
- Support and work with participants to find suitable supports and services that are the best match for them to actively pursue their identified goals and aspirations.
- Support participants to maintain appropriate funding and manage the plan review and renewal process.
- Promote United to Care to market and generating interest in support coordination under the NDIS. This will involve building relationships with various organisation’s such as Local Area Coordinators, Disability Service Providers and support groups including participants and nominees.
- Maintain and develop relations with quasi-government bodies and disability associations/groups.
- Research, manage and navigate a range of opportunities to suit individual support needs across multiple providers.
- Demonstrate a consistent high quality in written and verbal communication. Strong comprehensive report writing skills is a must.
- Participate in Quality Improvement activity, in relation to the quality management system.

**Qualifications and Experience**
- Certificate III or IV or above in Community Services, Disability support or similar demonstrated experience.
- Extensive experience in similar roles and in developing and implementing person-centred approaches that support people to build their capacity for choice and control over their own lives.
- Demonstrated negotiation, conflict resolution, communication, and mediation skills
- Ability to build positive relationships, communicate with and advocate for people of diverse backgrounds and abilities.
- Demonstrated capacity to work flexibly, possessing the ability to manage competing demands, and able to effectively set boundaries and limits where required.
- Current NSW Driver’s licence
- Current NDIS Worker Screening Check
- Working With Children Check
- Approved National Criminal Record check conducted within the last 6 months.
