**About the Role**

As an Aged Care Home & Community Assistant, you will be providing personalised support to older Australians to remain independent within their home whilst making a positive impact in their community. This role supports clients in our Transitionary Aged Care Program, providing care to older people returning home after hospital stays.

**Key Responsibilities**
- Personal Care including grooming and showering
- Domestic assistance such as meal preparation and light house cleaning
- Community access to medical appointments and social activities
- Supporting with tasks and routines
- Build positive relationships with clients

**Skills & Experience**
- Passionate about making a positive impact in our ageing community
- Certificate III in Individual Support (Ageing, Disability or similar) will be highly regarded
- An Australian Driver’s Licence and reliable vehicle with minimum third-party insurance.
- Strong communication skills with the ability to build and maintain positive relationships
- A current First Aid and CPR Certificate (must be obtained prior to employment)
