Key Responsibilities:
**Develop personalized support plans in collaboration with participants, focusing on their individual goals and aspirations.**

Provide direct assistance to participants in various daily activities, including personal care, social engagement, and skill development.

**Facilitate community integration and participation by organizing and accompanying participants to various events and activities.**

Collaborate with families, carers, and other professionals to ensure holistic and coordinated support for participants.

**Maintain accurate and up-to-date documentation in compliance with organizational standards and regulatory requirements.**

Participate in ongoing training and professional development activities to enhance skills and knowledge in disability services.

Essential Criteria:
**Diploma qualification in Disability Services, Community Services, or a related field.** Demonstrated experience in supporting individuals with disabilities, including the development and implementation of individualized support plans.

**Excellent communication and interpersonal skills, with the ability to build rapport and establish trusting relationships with participants and their support networks.**

Strong organizational abilities and the capacity to manage multiple tasks efficiently.

**Commitment to promoting social inclusion and advocating for the rights and dignity of individuals with disabilities.**

Valid driver's license and access to reliable transportation.

**Current First Aid Certificate and Working with Children Check.**
