**Attainable Care** is a leading Registered NDIS Provider dedicated to creating a world of possibilities for individuals with disabilities. We are dedicated to providing a range of tailored services, including **Supported Independent Living** **(SIL) and Specialist Disability Accommodation (SDA) homes**. We are currently seeking passionate and committed Support Workers to join our growing team, with multiple locations across Melbourne:

- South East Melbourne

**As a Support Worker with Attainable Care, you will deliver a range of services, including but not limited to**:

- _**Provide personalized support and assistance to NDIS participants, focusing on their individual needs, goals, and preferences.**_
- _**Develop and maintain strong relationships with participants, their families, and other stakeholders to ensure effective communication and collaboration.**_
- _**Implement and regularly review individualized support plans, adjusting strategies as required to promote participants' progress and well-being.**_
- _**Assist with daily living activities, personal care, skill development, and community participation, promoting independence and social inclusion.**_
- _**Monitor and report on participants' progress and any changes in their needs, ensuring timely and accurate documentation.**_
- _**Adhere to all relevant legislation, regulations, and organizational policies, maintaining the highest standards of professionalism and confidentiality.**_

**To be considered for this role, you must have**:

- _**Current NDIS Worker check, Police Check, and Working with Children's Check.**_
- _**Certificate of NDIS Worker Orientation Program.**_
- _**A current First Aid Certificate and CPR certification.**_
- _**A valid Australian Driver’s License & Reliable Car.**_
- _**A minimum of a Certificate III in Individual Support, Disability, or a related field.**_
- _**Experience working with people with Mental health disability and challenging behaviour.**_
- _**Previous experience in a similar role, preferably within the NDIS framework.**_
- **Ability to provide 2 current/recent work-related references.**:

- _**Strong interpersonal and communication skills, with the ability to build rapport and trust with diverse clients.**_
- _**A compassionate, empathetic, and patient approach to supporting individuals with disabilities.**_

Pay: $35.00 - $40.00 per hour
