Your responsibilities will include:

- Interviewing clients to understand their needs and assess difficulties they face
- Preparing plans for clients advising on support programs and implementing all aspects of the

plan, including informal, mainstream, community, and funded support.
- Following up with clients and monitor their progress and offer ongoing support and

guidance to participants to ensure their goals are met
- Referring participants to appropriate service providers and support families by providing

education on available services in the community.
- Assessing needs of the wider community for access to resources such as health, welfare,

housing, employment and other facilities.
- Liaising with community agencies about issues and promoting services/resources offered
- Preparing submissions and various reports for management and other agencies, including

implementation, progress, and end-of-plan reports.
- Diploma level qualification OR 3 years of work experience in lieu.
- You must also have at least 1 year of work experience in the field

The salary for the role is $79,040.

Pay: From $79,040.00 per year

Schedule:

- 8 hour shift
