**About the Role**
If you are motivated and passionate about positively contributing to the quality of life of our participants and supporting them to achieve their individual goals, then this role is for you The role will include:
- Engaging and coordinating service providers on behalf of Participants, as well as NDIA staff and Local Area Coordinators
- Engaging with Participants to develop and refine their individual NDIS goals, explain how to use their NDIS plans effectively to achieve their goals, and coach them in capacity building and increasing their independence
- Communicate with a Participants formal and informal supports to help facilitate achievement of a Participants goals
- Manage resources effectively and monitor the Participants progress achieving their individual goals to get the best outcome from their plan
- Teach Participants how to access the NDIS portal so they can monitor how their funds are being spent
- Assist Participants to prepare for their next NDIS review meeting
- Maintaining accurate and up to date case notes on Participants needs and progress
- Maintain clear professional boundaries with Participants
- Psychosocial coaching training available as future scope for the Support Coordination team
**Essential Criteria**
- Minimum two years demonstrated experience in a similar role.
- Relevant tertiary qualification or similar e.g., Certificate III in Disability, Individual Support or Diploma Community Services.
- Current WWCC, NDIS Worker Screening Check, NSW Driver’s License, and access to reliable, registered and comprehensively insured motor vehicle
- Proficient understanding of the National Disability Standards and NDIS operating environment, including support types and processes
- Demonstrated time management skills and problem
- solving skills
- Effective stakeholder engagement and communication skills
- Ability to effectively perform in a challenging and changing environment
- Advanced written and verbal communication skills, including use of computer software
**Desirable Criteria**:
- Degree qualification in Nursing, Community Services, Social Work or Social Science highly regarded
- Mental Health training or support experience
