**The Position**

Position Title: Social Worker
Classification: Grade 1 to Grade 3
Hours per fortnight: up to 80 with an ADO every 4 weeks
Employment Status: Ongoing and Fixed Term/Part Time and Full Time
Other requirements: COVID-19 Vaccination

We have positions available within the below departments:

- Transition Care Program Grade 2
- Outpatient Rehabilitation Services Grade 2
- Hospital Admission Risk Program Grade 2
- Inpatient Units Grade 1, Grade 2 and Grade 3

**Your New Role**

The Social Worker will be expected to provide information, support and counselling to patients and their families/carers as they adjust to their health condition, cope with changed health needs and determine the most appropriate care to meet their needs.

**Your Team**

The Clinical Operations Division encompasses acute, allied health, cancer and mental health services. We provide a wide range of general medical, surgical and specialty services including but not limited to; Oncology, Cardiology, Renal, Emergency, Women’s and Children’s, Critical Care, Specialist Clinics, Cancer Clinics and Mental Health Services. Our Allied Health teams provide a diverse range of programs and person centred care in inpatient, outpatient, community, home and residential care settings.
