We have a newly created opportunity for a permanent, full time **Aged Care Navigation Specialist** to join our Residential Operations team. Reporting to the Residential Funding Manager, this position is based at our Pier Street office. You will be required to be **available 5 days a week, 9:00am - 5:00pm (start and finish time can be flexible).**
**About the role**
The Aged Cared Navigation Specialist (ACNS) will be empowered to provide information, support and advocacy to all customers as they navigate the complex aged care system. The ACNS works with customers to understand their needs, identify available options, and make informed choices about their care. Duties will include:
- Provide customers the information and assistance they require about the aged care system, including eligibility requirements, services and supports and accessing My Aged Care Service and Support Portal to support informed decision-making regarding the delivery of their care and services
- Manage waitlists and vacancy boards in Epicor, including working closely with Village and Facility Managers to look at Home Care as an interim option for customers in the waitlist
- Monitor and communicate changed in legislation that affect subsidies
- Engage with internal teams to facilitate the resident on-boarding process, developing relationships with external parties including from admissions coordinators to case managers and other healthcare professionals
- Review the NSAF and / or Integrated Assessment Tool, including identifying the prospective AN-ACC classification
**About you**:
You are a highly experienced customer service professional and bring a strong understanding of the aged care industry. You can relate to a diverse range of stakeholders and have:
- Ability to develop relationships with our residents, their families/ representatives, and staff
- Excellent customer service skills, a passion for the wants, desires and need of older Australians
- Excellent verbal and written communication skills
- Strong understanding of MS Office suite of products
- Ability to work independently and autonomously, but also as part of a team
- Knowledge of Medicare claim, AN-ACC classification, My Aged Care Service and Support Portal is desirable
- Willingness to uphold the Mission, Vision, and Values of The Bethanie Group Inc
- Willing to obtain required vaccinations - COVID 19 (minimum of 3 doses) and annual flu vaccination
