Join the Parkerville Children and Youth Care Early Intervention and Prevention Team based in Midland as a Child and Family Care Navigator. This is an exciting opportunity to be part of our Family Support Network Program by delivering supports through the Assessment and Coordination (A&C) service offer, providing integrated trauma-informed, strength-based support to families, fostering an environment of empowerment and trust.
As a Child and Family Care Navigator, you will provide intake and assessment, therapeutic interventions, facilitate group work, and provide both in-reach and outreach client appointments. You will empower families by building on their strengths to overcome difficulties, offering practical and emotional support to children, young people, and their families experiencing mild to moderate mental health challenges.
This role is offered on a Permanent, full-time or part-time basis. It will primarily be based at the Midland location.
If you would be keen to join us, we are now seeking a passionate and committed professional to join our team who can:
• Provide intake and assessment, phone based therapeutic interventions, and facilitate group work.
• Offer one-to-one phone support, education, appropriate modelling techniques, and other supportive services.
• Provide practical and emotional support to families, fostering an environment of empowerment and trust.
• Build rapport and trusting relationships families
• Engage with families to achieve case plan goals
• Maintain proactive contact with children, young people, and families throughout their involvement with the service.
• Participate in multidisciplinary team case review meetings and facilitate the development of Individual Care Plans.
• Advocate on behalf of families to ensure they receive the necessary services.
