**Support Worker Job Description**

**JOB TITLE**: _Support Worker_

**JOB DESCRIPTION**:_The Disability Support Worker is responsible for the provision of_
- person/ family centered support services that empower people with disability to achieve their potential and live the life they choose. Disability Support Worker works under limited supervision with readily available assistance._
- Employees need to demonstrate Person-Centered and positive approach to supporting and empowering customers._

**TYPE OF EMPLOYMENT**: _Casual_

**REPORT TO**: _Mulki Ali_

**LOCATION**: _Mount Gambier_

**MAIN DUTIES/RESPONSIBILITIES**
- _Support clients to participate in age-appropriate activities and tasks including employment, in home support and community access._
- _Support clients to access activities and outings in the community._
- _Provide direct care assistance to each client inaccordance with their service plan and as directed by the Manager._
- _Work in accordance with the medication administration policy, work health and safety procedures and client rights policies including privacy and confidentiality._
- _Monitor the safety and well-being of the clients and report any concerns to the Manager._
- _Provide 1:1 or small group training and support in response to individual client needs._
- _Ensure completion of appropriate documentation._
- _Relevant information on support provided is to be recorded in client notes and communication book as required._

**QUALIFICATIONS/REQUIREMENTS**:

- _A cleared Working with Children Check and National Criminal Record Clearance_
- _Certificate III in Disability or other relevant qualification or working towards obtaining qualification within 12months (Desirable)_
- _NDIS Worker Module_
- _NDIS Worker Screening_
- _Valid Driver’s License_

**EXPERIENCE**:

- _Experience in the delivery of personal care services and/or challenging behaviors is viewed favorably_
- _Genuine desire to provide the best quality support for people with disabilities._

**SKILLS**:

- _Reliable character._
- _Well-developed communication and interpersonal skills including the ability to work and communicate effectively with clients, their families, community agencies and other professionals._
- _Ability to work in a very busy environment and to work both independently and as a part of a team._

**Job Type**: Casual

**Salary**: $30.00 - $60.00 per hour
