As the Community Network Consultation Clinician (CNCC) for Monash Health, you will share in the oversight of developing and leading this network to improve the identification and linkage of vulnerable children, young people and their families to the appropriate support service, supporting overall improved mental health outcomes.
The CNCC for Monash Health ELMHS will sit within the Intake, Assessment, Consultation and Brief Treatment (iACT) Team. The CNCC role is that of a senior clinician with training, qualifications and experience in assessing and treating young people with mental health problems. In summary the CNCC will be responsible for:
- Leading in stakeholder networking, including DET, DHHS and mental health services, for service information sharing and enhanced collaboration with the purpose of engaging vulnerable children, young people and their families in appropriate support services form early intervention through to specialist services. This committee will be chaired by the Community Mental Health Services Manager.
- Planning, implementing and evaluating mental health promotion activities that support capacity building within the community - fostering overall mental health in young people and enhancing skills and confidence of professionals working in the community to manage and support young people with mental health problems.
- Sharing in the coordination of all regular community consultation arrangements and once-off consultation requests (primary, secondary and tertiary in nature) within iACT and across the broader service directorate.
The iACT Team Manager and Consultant Psychiatrist are responsible for the clinical governance of the iACT team, whilst the Community Mental Health Service Manager will have operational governance responsibilities of the iACT team, of which the CNCC will be a member of. As such, the CNCC is accountable to these lead positions, in their respective areas of clinical or operational activities. It is the expectation that the CNCC will participate in team duties such as meetings (local and service wide), specific duty rosters, and hold a portfolio. All Monash Health clinicians need to adhere to the protocols and structures established in their local area.
