Our client, My Way Disability and Aged Care is a trusted provider of NDIS and Aged Care services, dedicated to empowering individuals to lead fulfilling lives. They deliver a wide range of services, including daily living support, psychosocial recovery coaching, allied health services, and community nursing care. With a strong commitment to providing high-quality care and support, they strive to make a meaningful difference in the lives of clients.

**Looking For**:
**Key Responsibilities**:

- Assess clients' needs and create individualized care and support programs.
- Develop and implement training and educational activities to enhance clients' skills and independence.
- Conduct client interviews to identify challenges and provide tailored solutions.
- Monitor client progress, prepare detailed reports, and communicate updates to stakeholders.
- Refer clients to appropriate external agencies for additional assistance and specialized care.
- Assess community needs and resources to enhance access to health, welfare, housing, and training services.
- Collaborate with community groups, welfare organizations, and government agencies to address client needs and raise awareness of available resources.
- Provide direct care and support for children, families, and individuals with disabilities in group housing and service units.
- Assist young individuals in overcoming social, emotional, and financial difficulties to promote personal development.
- Prepare funding proposals, resource requests, and formal reports to secure support from relevant agencies.
- Supervise and guide clients in compliance with safety and care standards, fostering a positive and nurturing environment.

**Skills and Attributes: -**
- Diploma, Advance diploma or Bachelor's degree in a related field.
- Minimum 1-2 years’ experience working in community services or a similar field.
- Knowledge of relevant legislation and policies related to residential care services.
- Ability to work collaboratively with clients, families, and a multidisciplinary team.
- Empathy, patience, and a genuine commitment to promoting the rights and well-being of individuals with disabilities.
- Strong interpersonal and communication skills with a compassionate and empathetic approach.
- Ability to work both independently and as part of a multidisciplinary team.
- Knowledge of NDIS processes and compliance requirements is an advantage.

**Job Type & Salary: -**
- This is a full-time permanent job.
- The base salary will be AUD 75,000- AUD 80,000 plus superannuation.

**Working Hours: -**
- Availability to work during evenings, weekends, and holidays as required.
