** The Role**

As Care Manager, you’ll play a pivotal role by delivering exceptional person-centred care management to our elderly clients in their own home. You’ll be more than just a case manager - you’ll be an advocate, problem-solver, and partner in helping our elderly clients live fulfilling, independent lives.

** What You’ll Be Doing**
- Work closely with Support at Home recipients to design care plans that prioritise independence, well-being, and dignity.
- Collaborate with our Care Companions and internal teams to ensure the seamless delivery of high-quality care.
- Connect with clients, families, and stakeholders to foster trust, engagement, and exceptional outcomes.
- Identify risk factors, engage in a co-management approach to care planning ensuring appropriate referral of clients to specialised services.
- Ensure integrity of client related documentation and data entry, maintaining accurate and timely records in Age Up Health’s client care management systems

- A valid Driver’s Licence and a reliable vehicle are required for this role. Regular travel within the East, South-East and Inner Metro region with occasional travel to the Northwest region. Travel is reimbursed.
