**About the Role**

Reporting to the NDIS Service Excellence Manager, you’ll be the key point of connection for NDIS participants, helping them achieve their goals and live more independently, while ensuring the highest standard of care and compliance. It's more than just a job - it's a career with purpose.
This is a **Permanent, full time**career opportunity based at our **Carrum Downs** office with work from home flexibility and travel to visit clients as required.
**What you'll be doing**
- Manage a meaningful caseload of NDIS participants - tailor services to meet their needs, goals, and preferences
- Conduct home visits and participant reviews
- Provide ongoing assessments and adapt care plans as needed
- Maintaining all participant-related documentation
- Collaborate closely with internal teams and external partners to deliver the best outcomes
**About You**
You are a passionate and self-driven professional keen to continue developing your skill set. You are well-versed in NDIS compliance requirements and have worked in a coordination (or similar) role. You have a genuine desire to provide the best quality service to all stakeholders and are comfortable dealing with a range of priorities. Your information technology skills are evolving, and you are comfortable adapting to new systems and processes. Ultimately, you enjoy being challenged and enjoy being part of a positive working culture.
