This organisation is looking for a Care Manager to join their amazing Aged Care Facility based in the Eastern Suburbs.

You will take on the role of a 2IC to the Facility Manager as you have this brilliant opportunity to learn from them.

As Care Manager, you will be responsible for coordinating services to maintain a home-like environment for residents. You will set the pace and be an exceptional role model for your team.

For your residents, you will advocate for the best care possible on their behalf and manage relationships with residents and their families with ease. You will be a proactive person and can manage and de-escalate complaints by fostering two-way communication streams.

You will be part of a team of two Case Managers reporting directly to the General/Facility Manager as well as a wider Care Team including Allied Health Professionals, Registered Nurses, Team Leaders and Quality Assurance personnel. Other tasks and duties will include:

- Responsible for the care of up to 57 residents when facility is fully occupied
- Oversee and coordinate resident assessment and care planning / case management processes and documentation
- Working collaboratively to support the General Manager in the formulation and development of operational plans
- Monitoring revenue and expenditure and take corrective action when required. You will also assist with setting KPIs for funding
- Ensuring the facility is operating in accordance with legal and ethical requirements
- Work with and support the ACFI Coordinator with submission of ACFI uplifts, appraisals, and reappraisals
- Adhere to infection control policies and procedures in accordance with best practice guidelines and legislation.

**About You**
- You are an operational Care Manager who takes initiative and is able to take charge
- You have extensive Care Management and Operations experience in an Aged Care Facility
- You are a demonstrated leader and work to high quality standards in everything you do
- You are critical thinker who can make decisions and advocate for your residents and staff
- You are AHPRA registered and have Tertiary Nursing Qualifications
- You enjoy being a part of a friendly, motivated and highly successful & caring team
