**About the Role**
We are passionate about providing meaningful support to people with physical and psychosocial disabilities You will respect the clients’ cultures, privacy, and dignity throughout your duties in this role.
As such, you will provide assertive outreach support to our cohort of clients to achieve independence and develop professional friendships within their homes and communities.
You will be responsible for supporting and motivating them to positively improve their physical and mental health, develop connections with the community and achieve their goals. Your role also includes promoting and demonstrating positive behaviour, learning experiences, and skill development to your clients.
**Your key duties**:
These include, but are not limited to:
- High-intensity Personal Care and Activities
- Activities of Daily Living
- Community Participation and activities
- domestic and Household Tasks
- Medication administration duties.
- Documentation and incident report
**Qualifications & experience**:
- NDIS Mandatory and Screening Requirements
- Working with Children Check
- Comprehensive car insurance
- First Aid & CPR Certificates
- Driver’s Licence (minimum P2)
- Certificate IV in Disability/Mental Health/Aged Care (desirable)
