Hayee Group Services Pty Ltd is registered NDIS service provider. We are currently seeking an experienced NDIS Support Coordinator to join our team located at our Head Office in Liverpool NSW 2170.

Support Coordinator will be responsible for:

- Delivering quality services and building rapport with NDIS participants and their families
- Supporting participants to understand their plan and supports available to them
- Coordinating and facilitating supports and services for participants in order to achieve their goals
- Coaching participants and family members on related matters
- Identifying any risks or barriers and providing solutions
- build and maintain strong relationships with all stakeholders including government and non-government bodies, service providers, allied health professionals and practitioners.
- Prepare plan review reports
- Work with other team members to achieve participants goals.
- Demonstrated experience in a similar role with sound understanding of NDIS guidelines and practices - **this is essential**:

- Excellent communication and interpersonal skills
- Ability to problem-solve and overcome complex challenges and barriers
- Driver’s licence and access to own vehicle
- National Police Check
- NDIS Worker Check
- COVID19 Vaccination

**Job Types**: Full-time, Permanent, Subcontract

**Salary**: $70,000.00 - $100,000.00 per year

Ability to commute/relocate:

- Liverpool, NSW 2170: Reliably commute or planning to relocate before starting work (required)

Licence/Certification:

- Working with Children Check (preferred)
- Driver Licence (preferred)

Work Authorisation:

- Australia (preferred)
