**The Role**:
As an **Experienced Support Coordinator**, you will work directly with NDIS participants to:

- Understand and implement their NDIS plans
- Coordinate a range of supports to meet their individual goals
- Build capacity and independence
- Resolve barriers and manage service delivery challenges
- Monitor outcomes and progress toward goals
- Collaborate with families, providers, and community supports

**What You’ll Bring**:
Previous experience in Support Coordination (minimum 2 years)
Strong understanding of the NDIS framework and support systems
Background in disability, social work, mental health, or community services
Ability to work independently and manage competing priorities
Excellent written and verbal communication skills
Empathy, patience, and a commitment to person-centred practice
Current driver's licence and reliable transport
NDIS Worker Screening Check, WWCC, and relevant qualifications

**Job Types**: Full-time, Part-time, Casual

Pay: $61,100.36 - $102,288.00 per year

Expected hours: 38 per week
