Hornsby is one of our Robust sites supporting 9 clients residing in 1 SIL home that also consist of clients self contained units. We support people with complex mental health, psychosocial, intellectual and physical disabilities and high physical support needs. This site requires pro-active and reliable staff who have a love for building relationships, person centered care and making a difference
- We require staff to work on a 24/7 rotating roster so applicants must have full working rights and able to work more than 20 hours a week to be successful._

**Your role**:

- Promote choice and control and show eagerness to participate in our clients’ day to day routines, goals, and interests.
- Follow positive behaviour support and client care support plans as directed by clinical teams and site managers.
- Encourage capacity building to enhance clients’ current skills, such as hygiene, safe relationships, domestic duties, using public transport and accessing the community safely.
- Person centered active support
- Provide personal care and manual handling/ use of hoists with dignity and respect.
- To be competent in your computer/ IT skills and report writing

**Some of our requirements**:
We are aware there are quite a few compliance checks (listed below) for this role however, what’s important to us is that you have regular access to your own car and a P2 NSW Drivers License or higher and flexible availability across a 24/7 roster which may include, mornings (6am-2pm OR 7am-3pm), afternoons (2pm-10pm OR 3pm
- 11pm), active/sleepover nights (10pm-6am OR 11pm-7am).
