We have an exciting opportunity for an enthusiastic and organised Field staff Coordinator looking to make a real difference through their work at myHomecare in Erina.

**So, what does the role entail?**

No matter your role with the myHomecare Group, you’ll play a key part in supporting our clients and their families. With us, you’ll become a true leader in the in-home care space - and we’ve got the comprehensive training and development tools to get you there. You’ll be responsible for:

- Administration tasks including printing, collating client home folders. Data entry of client information, scanning, saving documents into the clients file.
- Maintain client records, agreements, and care plans, compile within the necessary timeframe.
- Schedule client home assessment.
- Attend client home assessment. Provide clients / carers with information about community resources and support services available.
- Undertake telephone welfare check at 3 months and action feedback appropriately.
- Undertake face to face client annual reviews and action feedback appropriately.
- Self-rostering client home visits.
- Complete WHS Assessment of clients home and action appropriately.
- Working within the CHSP (Commonwealth Home Support Programme) framework and programmes within the myHomecare Government Funding

**We are looking for someone with**:

- Can do attitude who is self-motivated and highly organised.
- Results driven, and proven problem-solving ability.
- Certificate III in Individual Support ( or similar ).
- Aged care / community care industry experience preferred.
- Existing knowledge on Home care Packages and Commonwealth Home Support Programs.
- High level of verbal and written communication skills.
- Excellent time management and planning skills with the ability to multitask.
- Intermediate MS Office computer skills.
- The right to work in Australia.
- National Police Clearance or willingness to obtain (the myHomecare Group will organise via WorkPro at no cost).
- Proof of Covid Vaccination.
