HomeCaring Central Coast is a community care company, a national provider of high-quality in-home care for the elderly and NDIS participants. The services are provided to those in the community that are vulnerable and in need which include: assistance with personal care, household duties, meal preparation, medication support, transport to appointments, shopping and social outings.

We are seeking a proactive, highly organised part time Case Manager /Assistant with initiative and drive to succeed.

Key Responsibilities

Successfully demonstrate skills in case management, knowledge of Home Care Packaged program guidelines and Aged Care legislative requirements.

Records management, including continuous quality management and improvement

Effectively demonstrate experience in negotiating individual consumer budgets and coordinating complex care services to meet consumer care needs.
Professional liaison with clients and carers
Coordination of community-based needs

Have proven experience in negotiating & marketing available services to consumers and carers

Oversee client intake, administration, paperwork

Able to sign new clients and Rostering independently

You will have independence and flexibility of hours, supporting and driving our sales processes, as well as liaising with clients.

To successfully fill this role, you will have:

- AHPRA Registration highly regarded
- Excellent verbal and written communication skills

**Job Types**: Full-time, Part-time, Casual

Pay: From $30.00 per hour

Schedule:

- Day shift

Supplementary Pay:

- Penalty rates

Ability to commute/relocate:

- Toukley, NSW 2263: Reliably commute or planning to relocate before starting work (preferred)

**Experience**:

- Case management: 1 year (preferred)

Licence/Certification:

- Driver Licence (preferred)

Work Authorisation:

- Australia (preferred)
