As a support worker, you will assist in developing and implementing a person centred customer plan, and will support customers to develop and enhance their wellbeing, in areas such as living skills, community engagement, financial and health service access, pro-social activities, personal care, employment, education and training.

This is a **Part-time** position at **West Wallsend** working **Monday - Sunday on a 24/7 rotating roster, which includes Sleepover shifts, **reporting to the Team Leader.

**What Is Expected of You**
- Collaborating with customers to develop individualised support plans that incorporate their identified wellbeing needs and goals
- Providing support to customers to improve their independence by assisting with living skills, transport, social activities, medication and personal care, as well as other support as required
- Supporting customers to explore their practical and complex needs
- Keeping customer records up to date on a daily basis using on line systems
- Developing and maintain professional relationships and networks with customers, families, caregivers and key stakeholders
- Seek out innovative ways to enhance the customer journey

**Your Ticket to Play**
- Technical literacy with proven ability to use Microsoft Office and systems for reporting
- Minimum Cert IV in Mental Health or Disability or equivalent industry experience
- Availability to work across a rotating roster
- Valid NSW Drivers C class licence, with ability to drive vehicles in licence class, including large cars and vans.
- Willingness to obtain an Australian Federal Police Check, WWCC and first aid certificate that meet New Horizons employment criteria prior to commencing with us

**Equipment Requirements and Employment Check Criteria**
- Australian Federal Police Check (Covers all of Australia) from the last 24 months
- Work Rights - Birth Certificate or Passport
- Valid NSW Drivers C class licence, with ability to drive vehicles in licence class, including large cars and vans
- NDIS Worker Screening Check - Required for working with NDIS Clients in NSW
- Worker Orientation Module ‘Quality, Safety and You’
- NSW Working with Children Check
- COVID-19 Vaccination in line with Health Orders
- First Aid Certificate
