Disability Support Worker

**Role Description**
A Disability Support Worker is responsible for providing assistance and guidance to individuals with disabilities, promoting their autonomy and self-determination. This role primarily involves supporting participants in developing skills, building confidence, and making informed decisions. DSW collaborates with healthcare professionals, therapists, and families to create personalised care plans that address specific needs and goals.
DSW ensures a holistic approach to care by integrating various services and resources to offer comprehensive support. Ultimately, DSW are accountable for enhancing the well-being and quality of life of individuals with disabilities through tailored assistance and advocacy, fostering independence and empowerment.
**Role Responsibilities**
**Individualised Support**
- Be familiar with the goals and preferences of the people we support.
- Connect the people we support into the communities of choice and various programs based on their interests.
- Understand all support requirements for the people we support.
- Be involved in the development, implementation, monitoring, and review of Person-Centred Plans using an individualised planning approach.
- Support people in mealtime management, including the use of the Menu Log ordering system, supported with mealtime assistance according to the person's dietary and mealtime health support requirements.
- Provide documented individualised support when preparing and administering medication, in accordance with Administration of Medication procedures.
- Support people to participate actively in their daily routines and activities such as cleaning, mealtime management and grocery/personal shopping. Provide the minimum support required for them to be able to successfully complete any task using an active support model.
- Support each person to continually learn and ensure current skills are maintained wherever possible.
- Where appropriate and necessary, assist or supervise the people we support with their personal care and physical assistance. Ensure that each person’s comfort and dignity is respected at all times. This may include assistance with hygiene assistance, personal care, physical assistance, mealtime management, mobility and cleanliness.
**Administration & Time Management**
- Complete all required documentation in a professional manner and within the required timeframes.
- Use the computers and resources where necessary for the above administration tasks.
- Be punctual, complete work hours and make efficient use of the time spent at work.

- Allied Health professionals and external stakeholders.
- NDIS and NDIS partners.

**Key Attributes**
**Qualifications**
- An entry level qualification of Certificate III in Disability or equivalent. Applicants without qualifications must be able to provide evidence of relevant work history to be considered for this role.
**Skills & Experience - Essential**
- An entry level qualification in Certificate III in Disability or equivalent.
- An understanding of the NDIS framework and Disability Act 2006.
- Demonstrated commitment and ability to work and communicate effectively with people with a disability.
- A positive attitude and an ability to respond creatively in providing the support necessary for people we support to achieve their goals.
- Demonstrated ability to work under limited supervision with a substantial level of accountability.
- Demonstrated ability to work either individually or in a team.
- Well-developed communication (written and verbal) interpersonal and numerical skills.
- Computer literate.
**Employee Responsibilities**
- Complete a NDIS Worker Screening Check prior to employment.
- Maintain a current Driver’s licence.
- For non-Australian Citizens, an appropriate Working Visa is required to be maintained.
- Complete the NDIS Quality & Safeguarding Orientation Module prior to employment.
- Understand emergency procedures, location of evacuation points and take all reasonable steps to ensure a safe environment always promoting Safety Culture awareness.
- Take reasonable care for their personal health and safety as well as that of other people who may be affected by their conduct in the workplace.
- Participate in annual performance development, training as required of their position.
- Minimise wastage of office materials and general waste through reduction, reuse or recycle principles.
- Comply with Equisent Disability Services Policies and Procedures as applicable to your role.
**Qualifications and Checks**

- Current First Aid & CPR certification.
