**About the role**

An opportunity exists within the Adult Community Mental Health Services in the Yarra Ranges Continuing Care Team for passionate, enthusiastic & experienced mental health clinicians with sound clinical knowledge and mental health experience.

The roles will partner with consumers, carers, families and other service providers to provide high-quality case management and discipline-specific mental health assessment, treatment, consultation and community linkage services within a recovery-oriented framework. This position will be at our clinic in Ringwood, providing centre-based and outreach services.

**About you**

To be successful in this role, you should possess the following:

- Qualifications and experience as specified in the position description, either as a Registered Nurse, Clinical Psychologist, Social Worker, or Occupational Therapist with appropriate registration.
- Experience in mental health practice and discipline-specific practices.
- Demonstrated work experience in mental health.
- Demonstrated ability to provide mental health assessments and interventions for consumers and their carers individually and in groups utilising a case management model.
- Demonstrated commitment to and ability to practice utilising the recovery approach in mental health treatment and support with a focus on working collaboratively with consumers and carers to promote self-determination and enable progress towards
- reaching personal recovery goals.
- Well-developed interpersonal, organisational, and communication skills.
- A solid commitment to high-quality care.
- The ability to work as part of a diverse team.
- Flexible and agile in responding to operational and clinical challenges and complexity, focusing on supporting consumers and their families/carers to address their goals in their recovery journey.
