- We are looking for an aged care health professional with community care experience necessary to be our** Care Manager. T**he Care Manager will join our other Care Managers and the current nursing team. Our services include aged and disability care, transitional care, hospital in the home and palliative care. We are currently seeking experienced Nurses who are enthusiastic and passionate about providing a caring, compassionate, respectful, and high-quality service. Our priority is to help our clients live empowered and independently at home.
**Qualifications**:
**_Essential Criteria:_**- Health Care professional, experienced in Community and Aged Care
- Sound understanding of the principles of My Aged Care, Aged Care Reforms and Consumer Directed Care
- Comprehensive Assessment and Case Management experience
- Experience in provision of care and services to people from culturally & linguistically diverse backgrounds- Commitment to work within RAH Mission, Vision and Values
- Capacity to take initiative and to be innovative
- Highly developed organisational, administrative and financial management skills with proven ability to determine priorities, coordinate activities and meet deadlines.
