**The Opportunity**

As the Care Partner - Community Services you will support participants in the Support at Home (SaH) and Commonwealth Home Support Program (CHSP) to live independently in their homes by providing Case Management and coordinating service delivery according to assessed needs and funding. Enjoy this variety-filled Full Time role working 8:00am - 4:00pm Monday to Friday onsite with our collaborative team, supporting participants to meet their goals and enable continued choice and independence.
- Responsibilities: _
- Delivering a high standard of case management & service coordination that is informative and responsive of individual goals and choices.
- Collaborating with participants and representatives to deliver an individual goal plan that supports assessed needs, goals and package resources.
- Working with the Clinical Care Partner to coordinate clinical care delivery within the quality framework.
- Ensuring that service delivery is within participant allocated service budget & managing unspent fund balances for the participant.
- Monitoring service outcomes, ensuring continuous improvement and client satisfaction, and that key KPIs are met
- Supporting all participants with navigating My Aged Care and aged care services as required.Participating in an on-call roster (remotely) to ensure timely service provision.

**About You**
- Essential Criteria: _
- Qualification in Aged Care, Community Services, or a related field; (or equivalent work experience).
- Knowledge of My Aged Care, Support At Home (SaH), Home Care Packages (HCP) and/or Commonwealth Home Support (CHSP) Programs.
- Demonstrated experience in coordinating care services, managing budgets, and working with diverse clients.
- Have an understanding of the incoming Strengthened Standards and Aged Care Act and related regulatory frameworks
- Excellent communication and interpersonal skills, with a compassionate and client-focused approach.
- Driver’s License
- First Aid Certificate
- Unrestricted right to work in Australia
- Completion of a pre-employment assessment and a National Criminal History check clearance during recruitment process
- Desirable _
- Case Management experiencePrevious work within a Case Management framework
