We are currently seeking a motivated and skilled Positive Behaviour Support Practitioner to join our team, with a focus on supporting clients through tailored behaviour support interventions.

**Role Overview**:
As a Community Worker specialising in Positive Behaviour Support, you will be responsible for assessing client needs, planning and implementing strategies to manage and improve their behavioural outcomes, and working collaboratively with clients, families, and other healthcare professionals to facilitate community integration and independence.

**Key Responsibilities**:

- Conduct comprehensive behavioural assessments and develop individualized behaviour support plans.
- Implement and monitor behaviour strategies, adjusting plans to meet client needs and ensure effectiveness.
- Provide ongoing support and training to clients’ families and caregivers.
- Collaborate with a multidisciplinary team to ensure holistic client support.
- Advocate for clients and facilitate their participation in community activities.
- Maintain accurate records and documentation in compliance with legal and organizational standards.
- Assess clients' needs and planning, developing and implementing educational, training and support programs that benefit not only specific clients but the community as a whole
- interviewing clients and assessing the nature and extent of difficulties
- monitoring and reporting on the progress of clients
- referring clients to agencies that can provide additional help
- assessing community need and resources for health, welfare, housing, employment, training and other facilities and services, and working collaboratively with community groups, welfare agencies, government bodies and private businesses about community issues and promoting awareness of community resources and services
- supporting families and providing education and care for children and disabled persons in adult service units, group housing and government institutions
- preparing submissions for funding and resources, and reports to government bodies and other agencies

**Essential Qualifications and Skills**
- Degree in Psychology, Social Work, applied behaviour analsysis or related field.
- Certification or significant experience in Positive Behaviour Support or a similar behavioural intervention framework.
- Excellent communication and interpersonal skills, with the ability to work effectively with diverse client groups.
- Proven ability to handle sensitive situations with compassion and discretion.
- Working with Children Check, NDIS Worker Screening, Criminal History Check
- Drivers Licence with nil restrictions

**Desirable**:

- Experience in crisis intervention and conflict resolution.
- Knowledge and experience working within the local community, knowledgedable regarding resources and services.
