Our focus on independence while offering a quality service to those who require assistance to remain at home. We work with you to assist in making the best choices that align with your goals.

We are looking for a passionate Disabilities Services Officer to join our dynamic and growing Adelaide team.

Tasks/Duties

Assessing participants' needs and support programs
Meeting with participants and assessing the nature of their difficulties
Monitoring and reporting on the progress of participants and their goals
Assessing participants community needs and resources for health, housing, employment, and services
Liaising with community groups, about community issues and promoting awareness of community resources and services available
Supporting families and providing education and care for children and disabled persons in community settings, or group housing
Supporting offenders on probation and parole within a community setting
Assisting young participants to solve social, emotional and financial problems
Mandatory reporting to government agencies
Writing Support Plans
Care team meetings

Qualifications and skill level:
Desirable:
AQF Associate Degree, Advanced Diploma or Diploma (ANZSCO Skill Level 2)

At least three years of relevant experience may substitute for the formal qualifications listed above. In some instances relevant experience and/or on-the-job training may be required in addition to the formal qualification.

NDIS Workers Screening Check - Desirable
Worker Orientation Module
Must have a Car and Driver's License
Must have full comprehensive car insurance.

**Salary**: $55,000.00 - $65,000.00 per year

- Monday to Friday
- Weekend availability

Ability to commute/relocate:

- St Agnes, SA 5097: Reliably commute or planning to relocate before starting work (required)
