**1. Position Description**
**JOB Title**:Disability Support Worker
**JOB TYPE**:Casual
**LOCATION**:Office and Clients site (Penrith Nepean Blue Mountain area)
**2. MAIN DUTIES/RESPONSIBILITIES **(but not limited to):
- Support clients to participate in age appropriate activities and tasks including employment, in home support and community access.
- Support clients to access activities and outings in the community.
- Provide direct assistance to each client in accordance with their service plan and as directed
- Work in accordance with the medication administration policy, work health and safety procedures and client right's policies including privacy and confidentiality.
- Monitor the safety and well-being of the clients and report any concerns to the Manager.
- Ensure completion of appropriate documentation. Relevant information on support provided is to be recorded in client notes and communication book as required.
- Work with clients, family/ carers, relevant case managers and other professionals to support the client's individual goals.
- In conjunction with other members of staff assist in developing, implementing and evaluating training and behaviour programs to attain identified individual goals.
- Ensure that all programming undertaken is appropriately maintained, documented and reviewed.
- Assume the role of key worker for designated clients, including the preparation of a monthly report on client progress.
- Liaise extensively with other staff to ensure the overall smooth and consistent running of the service.Report regularly to manager
- Actively contribute to a team environment through open communication, participation in regular staff meetings, planning processes and policy review.
- Provide back up and support to other staff, as necessary.
- Participate in Performance Development meetings to identify ongoing training and support opportunities.
- Notify the service if you are unable to work your rostered shifts, with as much notice as possible.
- Contribute to Case reviews with Employment Consultants and Facilitators.
- Take reasonable care for his or her own health and safety
- Take reasonable care that his/her acts or omissions do not adversely affect the health and safety of other persons.
- Report any quality and safety incident or hazards at work to their Manager or Supervisor.
- Carry out their roles and responsibilities as detailed in the relevant policies and processes.
- To transport and accompany service users to facilities in the local community, including appointments.
**3. Qualifications**
- Cert 4 Individual Support, Mental Health, Aged Care, Community Services, Disability Work or similar.
- Current National Police Check
- Working with Children Check.
- First Aid certificate and CPR
- NDIS Workers Orientation
- NDIS Workers Screening Check
- 100 Points of ID
**4. Competencies and experience**
- Previous experience in a similar role - Minimum of 12 months relevant experience
- Previous experience with complex care needs
- Previous experience supporting People with Disability.
- Demonstrated proficient written, verbal and interpersonal communication skills.
- Demonstrated ability to work with mínimal supervision, and collaboratively as a member of a multidisciplinary team.
- Ability to effectively prioritise and organise own work / caseload.
- Demonstrated commitment to professional development, quality improvement and evidence-based practice.
- Good at domestic duties.

**Salary**: $38.00 - $70.00 per hour
