**About the role**

The Dietitian provides Dietetic services that lead to improved clinical outcomes for the patients of the Central Adelaide Local Health Network (CALHN), within the CALHN Integrated Care setting.

Integrated Care is a growing and exciting area to work, serving as an important conduit between primary care and hospital services.

The service supports Integrated Care hospital avoidance, supported discharge and hospital substitution models. The role will promote and advocate a ‘home first’ approach, where multidisciplinary assessment and intervention will be delivered outside the acute hospital setting. The Dietitian will be base with the CALHN BRIGHT service, but will work across CALHN sites and within Integrated Care, as required and in line with clinical need.

The position requires extensive liaison and collaboration with health, social and other relevant services and the development of contemporary practice guidelines and pathways to optimise outcomes for clients with multiple complexities.

The Dietitian demonstrates a proficient level of professional knowledge and skill and participates in departmental educational, quality improvement and planning activities.

This position may be required to provide professional/clinical supervision to AHP1 Dietitians, Allied Health Assistants, undergraduate and post-graduate Dietitian students, and work experience students.

**Essential Requirements**:

- Current National Police Clearance (NPC)
- Immunisation screening as indicated in the role description. SA Health Services are required to implement the Addressing vaccine preventable disease: Occupational assessment, screening and vaccination policy in the workplace.

**Educational/Vocational Qualifications**
- Appropriate degree or equivalent qualification which gives eligibility for full membership of the Dietitians Association of Australia (DAA)
