**About the role**

Are you a dedicated Dietitian ready to make a difference in an innovative and multidisciplinary healthcare setting? As part of the CALHN Integrated Care team, you will provide exceptional dietetic services aimed at improving clinical outcomes for patients, with a focus on hospital avoidance, supported discharge, and hospital substitution models. With a “home first” approach, you will deliver comprehensive assessment and intervention outside the acute hospital setting, while working closely with the CALHN BRIGHT service and other CALHN sites as required. This role emphasises collaboration with health, social, and community services to support clients with complex needs, developing contemporary practice guidelines and pathways to optimise their care.

In this dynamic position, you will demonstrate advanced professional knowledge and skills, contributing to departmental education, quality improvement, and planning activities. Additionally, you will have the opportunity to mentor and supervise AHP1 Dietitians, Allied Health Assistants, and both undergraduate and postgraduate students, shaping the future of dietetic care. **About You**

We are looking for a dedicated and skilled Dietitian to join our team and make a meaningful impact on the lives of clients with complex needs. To excel in this role, you will demonstrate:

- **Qualifications and Membership**: An appropriate degree or equivalent qualification that ensures eligibility for full membership of the Dietitians Association of Australia (DAA).
- **Skills and Abilities**: Strong organisational skills, the ability to work independently and collaboratively in multidisciplinary teams, high-level communication and interpersonal skills, and a proactive approach to problem-solving and conflict resolution.
- **Experience**: Proven clinical experience in assessing and managing clients with complex needs, participation in quality improvement initiatives, and expertise in supervising and mentoring students.
- **Knowledge**: A comprehensive understanding of clinical nutrition, evidence-based practice, quality improvement principles, and community support services, along with an awareness of Work Health and Safety protocols.
