ECH is expanding and our Care Coordination team are Seeking 2 Care Coordinator Support Officers (CCSO) to join our team in the North East and West region, based at Henley Beach or Walkerville.
**The role**
As CCSO you will provide administrative and co-ordination support to the Care Coordinator team with a continual focus on improving process and delivering exceptional customer service.
The CCSO role will also include managing our Commonwealth Home Support Program (CHSP), clients and may also include managing a small client case load of home care package clients. You will work closely with our Home Care Package clients, their families, carers, and advocates. You will ensure all clients have a Care Plan that reflects their individual care and services needs to enable them to have the best life possible as they age in their own home.
You will be provided with full training and support including all equipment that is required to succeed in this role.

**What you’ll be doing**
- Provide Administration Support to the Care Coordination Team
- Manage client intake, assessment, and reviews
- Provide guidance to new and potential clients
- Establish service and equipment requests
- Provide exemplary customer service as the primary point of contact
- Assist with the coordination of service for our clients

**You will need**
- COVID-19 vaccination (including booster if eligible)
- Drivers Licence
**It’d also be helpful if you had**:
- A current Police Check
- Diploma in Community Service Coordination or Certificate 3 in Aged Care or equivalent
- Experience and/or knowledge of Home Care Packages
