Care Co-ordinator

This is an exciting opportunity for an experienced **Care Coordinator** to join our Care Finder program in providing assistance to people in the care finder population to understand and assess aged care services and connect with networks within the community.

Located in Campsie and available immediately, this is a fulfilling opportunity for someone looking to explore and establish different ways to engage with harder-to-access groups in the community who are unlikely to engage with the system.

**The Organisation**

Multicultural Care is a not-for-profit, secular, community organisation that specialises in providing in-home and community-based support services for frail-aged people, people with disabilities and their Carers from Culturally and Linguistically Diverse (CALD) backgrounds.

**The Role**

Work for a reputable NFP in the Care Finder Program to work on implementing a community outreach program through;
- Coordinating an assertive outreach approach supporting reach into local community which actively identifies and engages with potential clients.
- Networking to build effective relationships in the community to promote the Care Finder program and assist with reaching potential clients.
- Report client and staff complaints, risks, hazards, and feedback as per policies and procedures
- Provide reports to Engagement Program Coordinator and other nominated staff as identified.
- Continuously communicate any changes or updates related to their service ensuring all documentation is accurately recorded.

**About you**

To be successful in this role you will have:

- A Qualification in Social Work, Aged Care or Community Services or 1- 2 years relevant experience
- An ability to attain an understanding of the range of aged care supports and services.
- A commitment delivering a person-centred approach that respects and responds to each person’s individual needs, preferences, values, and life experiences.
- Experience and demonstrated ability to engage with older people experiencing issues related to housing, financial, health and care issues.
- Microsoft word, Excel, PowerPoint and Outlook.
- Fluency in any of the following community languages plus English: Arabic, or Vietnamese is a must have.
- NSW driver's licence and access to own car.
