Position Overview
- Lead HACC-PYP service shift into YourCH’s care model
- Work with teams and partners to shape service delivery
- Represent YourCH at key forums and networks
- Run working groups to drive system change
- Co-design improvements with clients and community

**To succeed in this role you will have**:

- Qualification in health, disability, aged care or related field
- 5+ years' experience in health, aged care or community services
- Proven project leadership and project management skills
- Skilled in building and strengthening partnerships
- Experience using co-design in planning and service delivery
