**Role Overview**:
This is an exciting opportunity to join UnitingSA’s Community Aged Care team as our Coordinator. Reporting to the Program Manager, the Coordinator is responsible for the individualised care, support and services for consumers including the coordination, scheduling and implementation in accordance with approved funding, guidelines, legislative requirements and organisational work instructions.
The Coordinator will manage care and support which enables the consumer to live safely and independently at home while maintaining wellness, safety and connectedness, in line with the consumers co-designed Care Plan and within the scope of practice of the role. The Coordinator will work in collaboration with the multidisciplinary team to deliver clinical care (as appropriate) and outcomes for consumers
**Key Responsibilities**
- Work in partnership with consumers, key stakeholders and their carer/advocate to co-design individualised care, support and services.
- Regularly make contact with consumers and review their goals, needs and preferences, and update Care Plans with changes.
- Adhere to funding, guidelines, legislation and organisational requirements relevant to the consumer’s program, package or funding.
- Actively participate in relevant meetings including: family meetings, multidisciplinary, team, staff, on call, program specific, planning etc.
- Regularly monitor personalised consumer budgets according to program and funding requirements, and organisational processes.
- Audit documentation of quality and systems, as directed by Clinical Nurse/Program Managers/Manager.
- Set realistic expectations for consumers of support they can receive within the scope of their services.
- Prepare, complete and upload relevant documentation when on boarding new consumers, upgrading or transferring consumers to other service providers.
- Manage complex consumer situations and/or funding requests with support from the multidisciplinary team and Program Managers.
- Work in partnership with Support Worker (Language Support) who deliver culturally appropriate and language specific groups.
**Skills & Experience**:
- Experience in individual assessment, referral, triage, case management and coordination within a community services environment (preferably aged care).
- Prior experience in co-designing Care Plans.
- Previous experience supporting people from Cultural & Linguistically Diverse (CALD) backgrounds.
- Experience in using electronic consumer management systems and reporting tools
- Demonstrated ability to effectively managing competing priorities to deliver outcomes to a high standard within agreed timeframes.
- Sound understanding and working knowledge of the Aged Care Quality Standards.
- Knowledge and experience about the My Aged Care system and pathways.
- Highly developed written and verbal communication, negotiation, conflict resolution and problem solving skills.
- Ability to communicate sensitively, openly and effectively with consumers, their families/carers, key stakeholders, and other staff.
- Excellent time management and organisational skills.
- Experience navigating My Aged Care system and portal.
- Experience of Advanced Care Directives and End of Life Care.
- Experience setting firm boundaries within the scope of the role, whilst validating the needs and preferences of consumers and of other key stakeholders.
- Experience in using electronic rostering systems and consumer databases.
