**About the role**
The Community Access Unit is a small yet dynamic team which processes referrals from internal and external sources for the Ambualtory Care and Community Service such as the Community Rehabilitation Program, and SACS (Subacute Ambulatory Care Servcies) Specialist.

This position will provide you with the opportunity to refine your clinical skills while allowing you to maintain patient-centred care and provide you with a holistic view of the care continuum, from receipt of referral, triage and acceptance to the clinic/s. You will expand your knowledge on the broad suite of services provided by Eastern Health's ambulatory care and community services and learn or build-on your current clinical triage skills in a supportive environment.

**About you**
To be successful in this role you will possess:(examples below)
- Current clinical qualification and registration as a Registered Nurse or in an Allied Health discipline
- Current registration with AHPRA
- Knowledge of patient pathways through the acute, subacute, ambulatory and community service
- Highly developed interpersonal and leadership experience
- A strong commitment to high quality care
- The ability to work as part of a diverse team
- Excellent communication and organisational skills

Vaccination against infectious disease is a mandatory requirement of this role. An offer of employment is conditional on you providing evidence that you are currently vaccinated against COVID-19 with booster & Influenza, prior to commencing employment.
