Unify Disability Services is an organisation which provides various allied, mental and community health services to individuals with disabilities all across Melbourne. We are looking to expand our Community Support Team by recruiting a Disability Support Worker for an exciting, flexible and rewarding role.

What you will need:

- Experience in Personal Care & Manual Handling
- A Working With Children's Check (or willingness to acquire one)
- A Police Check (or willingness to acquire one)
- First Aid and CPR Certificate (or willingness to acquire one)
- A Valid Victorian Driver's license
- Experience in interacting with a variety of clients (preferred)
- Experience or educational background in the mental health, physical health, disability and aged care sector/field.

Position Description:

- (b) Undertaking associated duties as directed and in accordance with the client’s preferences, your (the employee’s) experience and professional judgement, and the client’s guardians/stakeholders;
- (c) Transport clients to and from their desired or required destinations. This may include, but is not limited to: activities, appointments, family, events, restaurants;
- (d) At times, you may be required to support the client with individual living skills which may entail the entirety or portion of the shift to be spent in their home/residence;
- (e) Supporting client’s in their areas of personal growth and enhance their quality of life. This includes but does not withstanding; supporting their use of specific recommendations or strategies recommended by their other practitioners, such as communication aids, enhancing their numeracy, linguistic and writing abilities, and other equipment within their home or external environment;
- (f) Ensuring that the provided support prioritises the safety of you (the Employee) and the client and is consistent with measures suggested by their carers, guardians or other stakeholders. This includes using effectively and monitoring the use of restrictive practices and issuing and medication required as routinely held for the client;
- (g) Participating in and completing relevant documentation. This includes but does not withstand; Person-centred plans, evaluations, skill development tools and reviews, writing progress notes about your interaction with the client after every session, which is uploaded to the nominated portal designated by Unify Disability Services, and may also be required to be written at client’s residency as a form of record keeping; and
- (h) Establish, maintain and respect the confidentiality of the client. This includes the sharing of client information or image on social media, face-to-face or by any other means, and unconcented disclosure of sensitive or private client information to any party.

Pay: $33.00 - $96.00 per hour

Schedule:

- Afternoon shift
- Day shift
- Monday to Friday
- Morning shift
- Public holidays
- Weekend availability

Supplementary Pay:

- Penalty rates
