**Integration & Empowerment Services (IES)** is a local not-for-profit based in Ellenbrook, Western Australia. IES is a registered NDIS service provider delivering a range of holistic support services, including premier in-home care, community and social supports, Recovery Coaching, respite assistance, facility-based and community-based social programs and more to residents of the Perth Metro area.
We are currently seeking _**EXPERIENCED SUPPORT WORKERS **_to join our dedicated team.
In this role, you will provide one-on-one support to NDIS participants from diverse backgrounds, working together to help meet their goals and needs.

**ABOUT THE ROLE**:
- Provide ongoing support to the organisations NDIS Participants using a person-centred, strengths-based approach to service delivery,
- Empower and support the organisations NDIS participants to live independently within their homes and in the wider community,
- Maintain accurate and up-to-date records and shift notes on the participants,
- Work within the scope of therole to ensure that the participant receives continuity of care,
- Work in close communication with organisation management regarding all supports provided to participants,

- At least one year experience working in a similar role,
- A passion for empowering others to meet their goals and needs,
- Ability to establish rapport with others,
- Honesty and Integrity,
- Maintain a high standard of professional, person-centered client support,
- Organisational and time management skills,
- Ability to work with a level of autonomy,
- Good communication skills,
- A knowledge and understanding of mental health and mental illness.
**REQUIRED QUALIFICATIONS**:
- Have a Certificate III + In industry relevant qualification
- Australian driver's licence with comprehensive car insurance
- First Aid Certificate
- Proof of COVID-19 vaccination
- NDIS Worker Screening Clearance (or willingness to apply)
- NDIS Worker Orientation Module
- Covid-19 Infection Control Certificate
- NDIS Code of Conduct Module
